According to the French National Authority for Health (HAS), more than 6,000 ankle sprains occur every day in France. In the face of this often-underestimated injury, there are a few essential steps to take in the first few minutes to limit the damage and prevent complications such aschronic ankle instability. What should you do immediately after a sprain? How can you assess its severity, and how can you properly rehabilitate the ankle to prevent recurrence? Here are our practical recommendations and the mistakes to avoid.

Key points to remember in case of an ankle sprain:
Immediately: Apply the R.I.C.E. method (Rest, Ice, Compression, Elevation) within the first 48 hours.
Do not apply heat, massage the area, or put weight on the ankle.Seek medical attention if pain persists after 48 hours, if there iswidespread bruising , or if you are unable to put weight on your foot.Do not neglect physical therapy: it is what prevents recurrence (up to 70% among athletes).

What is an ankle sprain?

An ankle sprain is a stretching or tearing of the ankle ligaments, most often caused by a sudden twisting motion: an inversion of the foot (the foot turns inward).An inversion sprain is defined by this forced inversion and affects one or more bundles of the lateral collateral ligament (external lateral ligament). It accounts for nearly 90% of ankle sprains. Less commonly, the sprain involves the medial collateral ligament or the tibiofibular ligaments, resulting from different mechanisms (eversion, external rotation of the foot).

How to Recognize an Ankle Sprain: Warning Signs

Before taking any action, it’s important to identify the symptoms so as not to underestimate the injury. The main signs to watch for are:

sharp, immediate pain concentrated on the outer side of the ankle, which worsens when weight is placed on it;

rapid swelling (edema) within minutes, caused by local inflammation;

difficulty walking or putting weight on the foot in cases of moderate to severe sprains;

the appearance of a hematoma after a few hours: a bruise indicating internal bleeding, which is common in moderate- to severe sprains.

The different levels of severity: the 3 grades

Grade 1: Strain of a bundle of the lateral collateral ligament, without rupture or tear. Moderate pain and swelling; the ankle remains mobile.

Grade 2: A tear in one ligament, with the others stretched or partially torn. Sharp pain, swollen ankle, possible bruising.

Grade 3: Complete tear of multiple ligaments, often accompanied by an audible popping sound. Severe pain; the ankle is completely unstable and cannot be moved.

When should you suspect a severe sprain?

If there is visible swelling of the ankle, an audible crack at the time of the injury, or a complete inability to move the ankle, you should see a doctor immediately. An X-ray will help rule out a related fracture.

The “R.I.C.E.” Rule: Immediate Steps to Take in Case of a Sprain

In the eventof an ankle sprain, the R.I.C.E. method (Rest, Ice, Compression, Elevation) is the standard first-aid approach. When applied promptly, it limitsinflammation, reduces pain, and promotes recovery.

R = Rest: Keep the ankle still and stop all activity. Continuing to walk may worsen the ligament injury. The HAS recommends avoiding putting weight on the ankle for the first few hours (usecrutches if necessary).

I = Ice: Apply ice for 15 to 20 minutes every 2 to 3 hours for the first 48 hours, always placing a cloth between the ice and the skin. Cryotherapy reduces local blood flow ,swelling, andbruising.

C = Compression: a tight (but not painful) elastic bandage, applied from the base of the toes up to the calf, to reduceswelling. Monitor circulation (tingling, bluish toes) and remove it before bed.

E = Elevation: Elevate the ankle above heart level several times a day to help drain the swelling. This is particularly effective when combined with ice.

Common Mistakes to Avoid When You Sprain Your Ankle

Do not consult a healthcare professional. A sprain that is not treated properly can lead to motor inhibition or rehabilitation setback, or chronic ankle instability , or recurrent injuries. Seek medical attention if pain persists after 48 hours, in cases of significant swelling, widespread bruising, an inability to put weight on the foot, or if you notice any deformity or a cracking sound.

Apply heat for the first 48 hours. Heat causes blood vessels to dilate and worsens swelling and bruising. Use ice instead.

Massaging or manipulating the ankle too soon. Massaging the injured area in the first few hours can make the injury worse. Wait for a professional to evaluate it.

Keep walking or put more weight on your ankle. Lack of rest impairs healing. Keep the area immobilized and off-load it for the first 24 to 48 hours.

Neglecting rehabilitation. Once the pain has subsided, the ankle remains fragile and prone to recurrence without appropriate rehabilitation (mobility, strengthening of stabilizing muscles, proprioception).

Causes and Risk Factors: Why Recurrence Is a Concern

Sprains are most often caused by accidents, but certain factors increase the risk (and especially the risk of recurrence):

the patient’s age, sex, and experience;

a history of ankle sprains;

a proprioceptive deficit in the ankle;

weakness in the muscles that move the ankle;

difficulty bending the ankle;

weakness in the hip muscles;

shoes that aren’t suitable for sports.

The primary risk, even for a mild Grade 1 sprain, is recurrence: among athletes, the recurrence rate can reach 70%, leading to chronic instability and early-onset osteoarthritis. This is the whole point of rehabilitation.

Why Rehabilitation Is Essential After a Sprain

After a sprain, rehabilitation is essential for regaining mobility, strength, and stability—and, above all, for reducing the risk of recurrence. The physical therapist focuses on six complementary areas:

joint mobility: gentle, manual mobilization techniques to restore lost range of motion before returning to weight-bearing activities;

Muscle strengthening: Several exercises can be performed gradually following an ankle sprain;

proprioception: rehabilitation of balance and control, key to preventing recurrence;

treatment of edema: manual drainage, cryotherapy, and pain-relief techniques;

pain management: often related to stiffness, which is why mobility is prioritized (TENS if needed);

Gait training: Correct limping and hesitation when putting weight on a limb, for example, in front of a mirror.

Galmeon Therapy in the Rehabilitation of Ankle Sprains

Galmeon therapy is a neuromotor reprogramming method based onmotor imagery, proprioception, and listening to low-frequency sounds emitted by a patented medical device. After a sprain, it helps relieve AMI (arthogenic muscle inhibitions) established by the brain to protect the joint and to progress through the stages of rehabilitation.

In practical terms, after a sprain, the peroneal muscles often exhibit inhibition on two levels: a loss of strength (excessively long response time) and an impairment in contraction sequences with the other stabilizers. However, upon heel-to-ground contact, the peroneal muscles must initiate a pre-contraction to ensure external ankle stability. Galmeon therapy aims to restore proprioception in the peroneal muscles (strength and function) and to reprogram these muscle activity sequences.Integrated into the physical therapy protocol, it helps accelerate treatment, optimize functional movements (a key concern for athletes), and provide a solution for patients suffering fromchronic instability that has not improved after one year. It enhances conventional rehabilitation without replacing it.

Find the Galmeon practitioner nearest you

Are you a professional?
